Getting Ready for Your Visit
In order for us to provide the best care for you, it is very important that you bring the following items to your visit:
- A list of your current medications
- Insurance card and information
- Medical records from your previous physician or your referring physician.
- The New Patient registration form
- Report for any imaging: ultrasounds, x-ray or CT scans.
You may bring the films if they are available.
