Our fees reflect the cost of providing your medical care. Health insurance is designed to help you meet the cost of your medical care, but the ultimate financial responsibility is yours. Your insurance contract defines to what extent the insurance company will reimburse you or will pay on your behalf. Our front office staff can assist you with your insurance questions.

If you have a change in insurance, please call the office to inform us. If you wait until your next visit, it may slip your mind, and you may be billed for services that your new insurance company would pay. You are responsible for payment for services that are not covered by your insurance, as well as any co-pays and deductibles.

ALL PAYMENTS ARE DUE AT THE TIME OF THE OFFICE VISIT WHEN THE SERVICES ARE RENDERED.
